myCitadel conducts due diligence on all new merchants as part of our Know Your Customer policy. All information gathered is kept in strict confidence in accordance with our Merchant Privacy Policy.
Instructions to filling out the Certified Merchant application
- Please answer each question.
- If a question is not applicable, please enter "N/A".
- If the space provided is insufficient, please use a separate sheet.
- Each Primary Executive or Shareholder with 10% or greater interest is required to complete the form.
- Individuals or corporations who hold shares in trust for a beneficial Primary Executive are not required to complete these forms.
- In instances where there are multiple Primary Executives or Beneficial Owners, please print a copy of the Application for each Primary Executive and submit as required.
- To help expedite the due diligence process and ensure timely integration, all information you provide must be accurate and complete.
- PASSPORT IDENTIFICATION IS REQUIRED FROM ALL PRIMARY EXECUTIVES. APPLICATIONS WILL NOT BE PROCESSED WITHOUT THIS INFORMATION.
How to submit the Application:
- Download the Certified Merchant application.
- Print the Primary Executive/Beneficial Owner Information Form from the downloaded document (one form for each Primary Executive).
- Print the Declaration of Ownership Form from the downloaded document (one form for each shareholder holding 10% or more of the business's shares).
- Ensure each Primary Executive and Major Shareholder fills in the forms completely (including both signatures for Primary Executives).
- Submit completed Primary Executive/Beneficial Owner Information Forms, Declaration of Ownership Forms, and Passports to myCitadel:

Citadel Commerce can not commence due diligence process until all required forms and passports have been properly completed and faxed.
Citadel Commerce, in its sole discretion, will determine whether to certify the merchant account.
While Citadel Commerce strives to complete due diligence within three business days, in some cases more time may be required.
